This bond is an accompaniment to the Streets Permit issued by the City for contractors to be able to do right-of-way installations, repairs, street openings, or other alterations. It ensures such projects being done on streets, sidewalks, curbs, parking lots, storm sewers, etc., will be completed and not left as a public nuisance or safety hazard. To prevent bond claims, contractors must complete construction according to the Chardon Code of Ordinances to restore the site to a usable state.

I, the undersigned, hereby apply for a Dishonesty Bond also known as a Business Service Bond or Janitorial Service Bond (“bond”) to the Surety Company (“SURETY”) through Jet Insurance Company (“JET”), with whom I hereby grant the authority to act on my behalf with respect to the bond and assign as my Broker of Record, and declare that the statements herein are true and correct.
